3.0 Credits Defines the role and functions of a case manager including thehistory, tasks, challenges of the job, and knowledge and skills necessary to be effective.Also,how to make referrals and act as an advocate and community resource specialist. Course-level Learning Objectives (CLOs) Upon successful completion of this course, students will be able to:
- Define the concept of case management.
- Define tasks or functions of a case manager and the key components of the strengths-based case management approach.
- Identify what kinds of knowledge and skills are necessary to effectively perform the case manager role.
- Identify the essential elements of a comprehensive individualized service or personal responsibility plan (also referred to as a case plan).
- Identify a basic process for resolving ethical dilemmas involving service delivery.
- Describe the process of making effective referrals.
