IHCM 340 - Care Coordination and Navigation
2.0 Credits
Examines the ways in which care coordination can support positive health outcomes, promote the quality of care, and improve cost efficiencies. Overview of healthcare models are incorporated into the larger framework of clinical care oversight. Special emphasis on coordination needs in senior services and behavioral health.
Prerequisite Admission to ICHM - BAS program.
Course-level Learning Objectives (CLOs)
Upon successful completion of this course, students will be able to:
- Explain the purpose of care coordination/care management and its correlation to patient outcomes.
- Assess current systems that create challenges in access to health services and compliance with care plans and develop strategies to work with community partners to overcome them.
- Identify methods to coordinate services and care transitions to meet client needs in general populations as well as special populations such as seniors and behavioral health clients.
